Waiver Program Coordinator - Worcester

MassAbility is seeking a dynamic individual to fill the role of Waiver Program Coordinator. The selected candidate is responsible for needs assessment, service plan development, service authorization activities, and implementation and monitoring of individualized service plans (ISP) for Acquired Brain Injury Non-Residential waiver (ABI-N) and Money Follows the Person - Community Living Waiver (MFP-CL) enrollees/participants statewide, as well as for MFP Demonstration participants. Waiver Program Coordinator engage in outreach as assigned to ensure that potential applicants are aware of the waivers and supported in the application process.
