Job summary
We are looking for an experienced Mental Health Practitioner from a background in Mental Health Nursing, Occupational Therapy, or Social Work to join our Brighton and Hove Neighbourhood Mental Health Team. As an experienced and Senior Mental Health Practitioner, you will have experience and knowledge of secondary care mental health services in order to support and oversee referrals to these services and identify appropriate pathways for service users in Brighton and Hove.
Main duties of the job
The post holder will work alongside colleagues and take a lead in the daily Multi-Disciplinary Team (MDT) triage meetings and the Multi-Agency Triage Hub (MATH) to identify pathways and access for referrals to mental health services.You will have excellent communication and organisational skills and will need to develop positive working relationships with our primary care and third sector colleagues in order to stream...
