St John of God Subiaco Hospital is seeking a skilled and compassionate Social Worker to join the hospital’s multidisciplinary Allied Health Team.
In this rewarding role, you will provide high-quality clinical social work services to patients with complex health, psychosocial and discharge planning needs. Working collaboratively with patients, families, carers, medical specialists and allied health professionals, you will play a vital role in supporting recovery, independence and wellbeing.
The role is offered on a Permanent Part Time basis, working 30.4 hours per week
The Position:
Deliver comprehensive social work assessment, intervention, counselling, advocacy and discharge planning to support patient-centred care and positive health outcomes.
