Job Description

This Social Worker II position provides quality social work services within the health care setting, in-home care, and supportive living sites. The role offers front‑line services to clients and families related to the social impacts of their health journey. Responsibilities include assessing social impacts, intervening with supportive counseling, accessing resources, assessing risk, and educating regarding the impacts of legislation and social determinants of health.

The role requires a broad knowledge of community and government resources, and the ability to implement various legislation that guides interactions with adults and seniors. Previous work in a hospital setting and working with seniors on the placement journey would benefit this position. The Social Worker must work well independently, as well as within an interdisciplinary team. Travel to rural sites within the Zone—including Bassano, Bow Island, and Oyen areas—is a requirement.
