Social Worker II – Brooks Health Centre, Social Work Unit & Program
Overview
This Social Worker II position provides quality social work services within the health care setting, in‑home care and supportive living sites. The role delivers front‑line services to clients and families related to the social impacts of their health journey, including assessment, counseling, resource access, risk evaluation and education on legislation and social determinants of health.
Responsibilities
- Conduct psychosocial assessments and resource‑based interventions.
- Provide individual, group and family treatment.
- Screen for risk, deliver counseling and manage cases.
- Coordinate transition or discharge planning.
- Engage in prevention, promotion and public awareness activities.
- Travel to and serve clients at Bassano, Bow Island and Oyen rural sites.
