Job Title: Social Worker – Hospital Team 
Location: Hull 
Pay Rate: £35.48 per hour
Liquid Personnel is recruiting an Adult Social Worker to join its client’s Hospital team, based in Hull.

What will your responsibilities be?
Assess patient needs to support emotional, social, and practical well-being during hospitalization.

Coordinate discharge planning by arranging follow-up care, rehabilitation, or community services.

Provide crisis intervention and emotional support to patients and families during medical emergencies.

Advocate for patient rights and help navigate healthcare systems, benefits, and legal protections.
