Social Worker – Care Plan & Resident Support Specialist

The Supreme HR Advisory Pte. Ltd. is recruiting for a full-time role based in Bukit Batok, Singapore. The successful candidate will develop individualised care plans for residents, execute the plans, monitor progress, and coordinate care with relevant stakeholders.

Requirements include a Degree in Social Work and preferably experience in the Social Service sector or related field. Salary ranges from SGD 3,500 to SGD 4,500 monthly, with standard local working hours.
