Family Service Association is seeking a part time (21 hours per week) Site Coordinator for our Countryside Adult Day Health Program in Raynham, MA. The Site Coordinator assists the Program Coordinator with management of day-to-day operations of the program, provides case management services, and monitors mental health status of program participants. The hourly rate is $21 and the hours are Monday through Friday, 10:00 a.m. - 2:00 p.m.
Duties and Responsibilities :
+ Provide case management services
+ Develop and lead groups for the participant.
+ Assist the program coordinator in referral, admission and discharge process.
+ Write progress notes quarterly.
+ Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.
