Job Description
Position: Hospice RN
Pay Rate: $55/hr
Location: Salisbury, MD
Schedule: 8a-4:30p (M-F), 40 hours per week
Contract Length: 13 Weeks
Coverage Areas:
+ Dorchester County
+ Wicomico/Worcester Counties
Key Responsibilities:
+ Deliver hospice patient care while maintaining accountability for clinical outcomes
+ Manage end-of-life care in patients' homes
+ Conduct initial evaluations within 48 hours of referral, initiate personalized plans, and collaborate daily with physicians and care teams
+ Provide critical symptom management, psychosocial support, and ensure meticulous clinical documentation
+ Exercise sound clinical judgment in an effective and efficient manner
+ Coordinate patient care with interdisciplinary team members and caregivers
