Join Stonehenge Therapeutic Community as a Registered Practical Nurse focused on supportive addiction and mental health housing. This role emphasizes direct client care, assessments, and multi-disciplinary collaboration in Guelph-Wellington.
As a critical member of the Supportive Addiction & Mental Health Housing program, the RPN will provide vital support in achieving housing stability for individuals facing addiction and mental health challenges. You will engage in holistic, client-centered care, ensuring best practices in both physical and mental health assessments. This is a mobile role requiring regular travel in the Guelph-Wellington area, working closely with communities and multidisciplinary teams.
Key Responsibilities:
• Conduct thorough health and risk assessments for clients
• Provide direct nursing interventions and treatments
• Manage medications and liaise with pharmacies
• Document care in accordance with CNO standards
