Registered Nurse (RN)

About the Role One Home Health is seeking an experienced home health nurse to support our Indiana team in a part-time capacity.

Openings are available in Lake County, South Bend, and the surrounding areas.  What You'll Do Conduct ongoing observation, assessment, and documentation of patient symptoms and progress.

Perform initial home care visits and reassess patient needs and progress as required.

Collaborate effectively with interdisciplinary team members to ensure coordinated care.

Demonstrate proficiency in accurate and timely OASIS documentation.

Provide supervision and guidance to Home Health Aide (HHA) field staff as appropriate.

Adhere to all applicable Medicare regulations and compliance standards.

Promptly notify the physician of any changes in the patient’s condition.

Coordinate care with other disciplines to support comprehensive patient needs.
