Position Overview
The RN will serve as a key liaison between patients, families, and providers to ensure a positive patient experience and continuity of care.
Key Responsibilities
+ Assess and monitor patient conditions.
+ Educate patients and families regarding treatments, medications, and follow-up care.
+ Coordinate communication between patients and providers.
+ Maintain accurate patient documentation and records.
+ Perform key administrative duties.
+ Provide telephone triage and patient support as needed.
+ Support care coordination and patient navigation throughout the healthcare system.
