Job Summary
Reporting directly to the Practice Manager and Senior Director of Nursing, the Team Nurse is responsible for performing RN/LPN duties in an outpatient primary care or family medicine clinic. This role serves as a key clinical resource for the medical practice and delivers high-quality, direct patient care within a team-based care model. The Team Nurse enhances the safety, effectiveness, and efficiency of patient care by supporting and delegating clinical and administrative responsibilities to Medical Assistant (MA) staff. This position also plays a critical role in patient education, care coordination, and supporting smooth transitions between levels of care. Key Responsibilities
