Proactive Care Coordinator (12-Month) – Home Visits

Kirklees Council is seeking a Care Coordinator to join the Personalised Care Team on a 12-month fixed-term project. The successful candidate will work independently with a 1-to-1 caseload, supporting individuals with multiple health conditions across Kirklees.

This role involves flexible hours, including home visits, and requires a DBS check. Candidates should have transport access and be ready to accommodate occasional evening or weekend work.
