Plan Writer
I. SUMMARY
Responsible for the development and review of all plans for individuals enrolled in and receiving Employment Services.
II. ESSENTIAL DUTIES AND RESPONSIBILITIES
? Reports to the Assistant Director of Employment Services. Maintains daily contact in person or by phone/email.
? Provides documentation in accordance with State and Federal regulations.
? Develops and maintains Employment Staff Action Plans for all individuals enrolled in Supported Employment Services, Community Prevocational Services and Pathway to Employment upon entry and every six months thereafter.
? Attends all semi and annual reviews for individuals receiving services and participates in other team meetings as necessary.
? Completes the individual's satisfaction survey in conjunction with the annual meeting and then reviews at each semiannual and annual thereafter.
