Summary: CHI Memorial Hospital is looking for locum CV Surgery coverage for the below dates to provide 24-hour call coverage across three different facilities (listed below).

Rates Caps for submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Holiday - Must be 1.5x the hourly rate*** Per holiday hour, no OT
On Call (Daily) - ***-*** - Per 24 hours to include 8 hours gratis
24 Hour Holiday Call Rate - Per *** hours to include 8 hours gratis
Call Back - ***-*** - Must match Hourly rate - Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Holiday Call Back - Must match Holiday rate - Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
