Job Title: Physical Therapist – Home Health/Hospice
Location: Escanaba, MI 49829
Duration: 3+ months with possibilities of extension
Shift: Monday–Friday, office hours 8:00 AM – 4:30 PM
Job Responsibilities
Physical Therapy admissions Add-on evaluations, reassessments, and discharges Create individualized plans of care for patients Supervise and work in conjunction with PTA Participate in weekly case conferences and monthly staff meetings Required Qualifications
Doctoral degree Home Health experience Certification Requirement
Physical Therapist License in MI Additional Information
Weekly hour guarantee: 32 hours Productivity expectations: 30 points weekly Coverage area: 1-hour radius around Escanaba Mileage reimbursed between visits (from first patient to last patient)
