Key Responsibilities
- Prioritise patient care by supporting patients with one or multiple medical conditions where medicines optimisation is required.
- Undertake clinical medication reviews, review monitoring needs and produce recommendations for senior clinical pharmacists, nurses and/or GPs on prescribing and ongoing care.
