At Understood Care, our mission is to help Medicare patients more easily navigate the healthcare system by providing access to tools, resources, and member-support services through our technology platform.
Responsibilities
- Serve as a member-facing contact after a member’s intake appointment
- Help members understand how to access healthcare services, find providers, and schedule appointments
- Assist members with navigation issues such as online portals, referrals, and application forms
- Use platform information to identify practical support needs and social determinants of health resources
- Coordinate with clinical professionals while providing only non-clinical navigation support
- Document member interactions and support activities through the platform
