This post is open to newly qualified professionals and those returning to practice, and we welcome applications from individuals at the start of their career or re-entering the workforce. https://youtu.be/AAw8fv32fP8 An exciting opportunity has arisen for a Band 5 Occupational Therapist post in the Carrick Home First Team providing assessment, reablement and rehabilitation to adults with health problems. This includes creating and reviewing care plans, reablement plans to facilitate independence and self-management and rehabilitation plans for support workers to implement. The teams support the Ageing Well Programme with 2 Hour Urgent Care Responses, 48 Hour Reablement Responses and Anticipatory Care Responses. The team plays a key role in supporting prevention of admission to Bedded Care and Discharge to Assess pathways.
