Become a vital part of Thornhill Community Health Centre as a Nurse Practitioner. This full-time position focuses on delivering comprehensive, client-centered care for underserved populations.
In this role, you will work collaboratively within an interdisciplinary team to provide extensive healthcare services that span assessment, diagnosis, treatment, and management of health conditions. You will particularly assist individuals facing barriers, including seniors and those with disabilities. Your contribution will help shape innovative, equitable healthcare solutions.
Key Responsibilities:
• Provide comprehensive primary care services
• Conduct health assessments for diverse clients
• Diagnose and manage chronic and acute conditions
• Collaborate with interdisciplinary health teams
• Document clinical findings in Electronic Medical Records
Requirements:
• Registered Nurse in Extended Class (RN-EC)
• Completion of recognized NP educational program
