Rates Caps for Submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly - *** and below - Per hour, not OT
Holiday - *** and below - Must be 1.5x the hourly rate*** Per holiday hour, no OT
