Become a vital part of Noojmowin Teg Health Centre as a Mobile Wellness Case Manager. Focus on providing comprehensive mental health services and supports across the diverse landscape of Manitoulin Island.
In this role, you will evaluate clients facing serious mental illnesses, creating tailored service plans in collaboration with caregivers and community resources. You’ll participate in interdisciplinary consultations while educating clients and families about available support services. Your strong clinical judgment and crisis intervention skills will ensure effective management of complex cases, fostering client independence and recovery.
