LPN Case Manager- Home Care

Home Instead® is looking for an energetic, dynamic, competitive and sales-oriented LPN to join our team as a Case Manager. If you have community home care experience, and are ready to use your skills in a real leadership role by guiding families through the Home Care Maze, this position might be for you. You will be relied upon by our clients and their families to help navigate the retirement and care options they are facing. The LPN Case Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/Care Pro introductions and quality assurance visits with existing clients.
