In this part-time role, you will implement and evaluate care plans while ensuring accurate documentation and assessments. Collaborate with the Director of Care and other professionals to schedule and facilitate care conferences, manage transitions, and provide direct care. Your expertise in nursing will help support residents and their families.
Key Responsibilities:
• Develop and assess resident care plans and outcomes
• Document all care according to regulatory policies
• Schedule residents' care conferences as needed
• Provide medication and treatment administration
• Collaborate in resident transitions for effective care
Requirements:
• Diploma in Practical Nursing with active provincial registration
• Minimum 3 years’ experience in senior care settings
