Description : The Licensed Practical Nurse works with other members of the client's care team to assess for, plan and provide integral health services within the client's home.

- Hold and maintain active license status as a Licensed Practical Nurse in the state of Minnesota
- Maintain a valid Driver's License and automobile insurance
