Job Description
Join our mission as a Program Coordinator overseeing the Housing First Program. Coordinate referrals, deliver mental health support, and work on community-based solutions in Aurora, Ontario.
This role involves collaborating with the Manager to ensure effective delivery of mental health and housing services. You'll be responsible for managing referrals, conducting needs assessments, and maintaining accurate records using CRMS. Additionally, you will support clients through counseling and case management, while collaborating with other professionals on treatment strategies.
Key Responsibilities:
• Monitor and manage incoming client referrals
• Consult on program capacity for workshops and events
• Conduct needs assessments and document client interactions
• Work with finance to track financial requests
