Licenses: Active Michigan medical license, Michigan controlled substance license DEA / CDS: DEA with Michigan address and schedules II, IIN, III, IIIN, IV, V. Certifications: ACLS (AHA only) Experience: Minimum 2 years recent experience; new graduates accepted via training program.
