Home Care Nurse

Will be responsible for the provision and maintenance of quality and safe clinical services to home care clients.

• Delivery of safe care and sound clinical decision making
• Assessing and documenting observations of client’s condition at each visit
• Initiating/reviewing nursing care plan based on assessment made
• Maintaining accurate nursing documentation
• Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.
• Conducting regular evaluation of care provided for home care services
• Provide caregiver training and education to client including families on home health care procedures and strategies
• Reporting of any problem incidents encountered in a client’s condition and/or in handling clients and/or caregiver to Nurse Lead
