Position Summary: The High Fidelity Wraparound Care Coordinator utilizes the Wraparound Model to help families with complex needs, providing comprehensive, coordinated, individualized, culturally-competent and cost effective community-based services that support children, youth and their families in order to enhance safety and maintain them in their homes, schools and communities.
