Enablement Support Worker

You will play a key role in the multi-disciplinary team assisting social workers to deliver the Local Authority's statutory duties to support the transition of individuals with learning disabilities and/or autism out of hospitals and into the community.

You will need to be able to communicate effectively both with the clients, their families and other professionals. Your role will include tasks such as enabling individuals to achieve their goals in life, supporting them to maximise their benefits and find opportunities to help them integrate into their community, undertaking reviews as per the Care Act 2014, supporting the individuals to find a suitable home for them in the community and attending some meetings such as ward rounds, Care Programme Approach meetings and Care and Treatment Reviews on behalf of the allocated social workers.
