Job Description
Responsibilities
- Provide emotional support and encouragement to patients and families experiencing behavioral health crises
- Handle case referrals, assess resourcing needs, and determine case closure
- Assist families in recovery goals through psychoeducation and community resource linkage
- Assess social determinants of health including housing, food insecurity, and transportation
- Connect families with support groups and mental wellness networks
- Collaborate with interdisciplinary teams to facilitate treatment plan goals
- Coordinate care with agencies such as DCFS and school systems
- Participate in scheduled consultations to monitor patient progress
Requirements
- Master’s degree in Social Work (MSW)
- 2+ years of case management or relevant experience
- Experience with acute behavioral health needs (suicidality, homicidality, severe persistent mental illness)
