Community Nurse

This vacancy is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board, Medway Community Healthcare, Student Nurses and Newly Qualified Nurses. The position does not meet the criteria for skilled worker sponsorship; we require documentary evidence of right to work in the United Kingdom.
We are looking for a compassionate, proactive and committed community-based nurse to support housebound adults (18+) with complex and long‑term conditions within our primary care network teams. You will work alongside GPs, mental health professionals, social workers and voluntary services to deliver holistic, patient‑centred care.
