Overview
We are seeking a motivated and dynamic Hospital at Home Community Matron to join our Neighbourhood Teams within the Barnsley Physical Health & Wellbeing Care Group. The role is central to delivering Hospital at Home pathways, working alongside Urgent Community Response, Crisis and Virtual Ward services, supporting acutely unwell patients in their own homes with consultant oversight and multidisciplinary team support. The Matron will also provide cross‑cover for proactive neighbourhood care of patients with complex comorbidities, contribute to Ageing Well services and multidisciplinary clinics, and help transform community‑based care in line with national priorities.
Service is delivered between 08:00 and 18:00, 365 days per year.
