Job Description
The Non-Clinical Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).
**Minimum Requirements:**
1. CHW Certification reuired
2. Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred
3. Current CPR if applicable
4. TB questionnaire, PPD or chest x-ray if applicable
5. Current Health certificate (per contract or state regulation)
6. Must meet all federal, state and local requirements
7. Must be at least 18 years of age
