PART TIME-DAYS-REMOTE
Scope of Work
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Qualifications
+ Bachelor’s degree or equivalent Required.
+ Master's degree or equivalent Preferred
+ 2 years of relevant experience required
+ LIC-Registered Nurse (RN) - STATE_MI State of Michigan Excludes CV Administration, HM Patient Care Logistics, Health Comm - Department, GM-Community Education, Lifestyle Medicine, PD HDVCH Child Protection, PH Medical Affairs Admin, PH Pre-Service Utility Management Upon Hire Required Or
