Clinical Pharmacist – Home Infusion
Position Overview
The Clinical Pharmacist manages patient care within a home‑infusion environment by processing orders, reviewing drug interactions, completing clinical assessments, coordinating with prescribers, and performing final product checks for quality control. The pharmacist will work closely with a small, collaborative team consisting of another pharmacist, two pharmacy technicians, and the general manager.
Key Responsibilities
+ Clinical Order Review — Review medication orders, assess drug interactions, and ensure clinical appropriateness.
+ Provider Communication — Contact prescribers for order clarification, updates, and ongoing patient needs.
+ Patient & Office Coordination — Communicate with MD offices and patients regarding continuation of service, discharge, or changes in therapy.
