Main duties of the job
Management of patients with chronic diseases to proactively manage patients with complex polypharmacy.
Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
Provision of specialist expertise in the use of medicines while helping to address public health and social care needs and tackling inequalities across Trafford.
Development of relationships and close working with pharmacy professionals across the wider health and social care economy.
Provision of leadership on person-centred medicines optimisation and quality improvement.
Delivery of first‑class, professional service to support patients and GP practice.
Proactive delivery of QoF and other locally enhanced targets.
