Job Summary

Specialist Clinical Pharmacist – Base at Darlington Memorial Hospital or University Hospital of North Durham. The successful applicant may be required to work at any Trust hospital site.

Responsibilities

• Deputise for the Pharmacist Advanced, supervising a team of clinical pharmacists, pharmacy technicians and support workers to ensure the delivery of safe, effective and efficient clinical pharmacy and medicines management services.
• Provide medicines management, optimisation and reconciliation services to designated wards, day‑case units or clinics as directed by the Lead Pharmacist.
• Develop and implement local policies and guidelines on good prescribing practice.
• Influence local practice to ensure safe medication use, recognising good prescribing practice, patient safety and risk minimisation.
