SUMMARY STATEMENT OF POSITION:

The Central Access Team provides one access point for referrals to all HopeLink Services and for all individuals inquiring about services. Under the direction of the Director of Central Access, the Central Access Clinician responds to referrals and inquiries for all HopeLink services, admission to services, and ensures the completion of initial and annual clinical needs assessments for all Medicaid-funded HopeLink programs.
