Description We are looking for a compassionate and organized Case Manager to support formerly homeless adults in maintaining stable housing and improving overall well-being in San Francisco, California. This Long-term Contract position focuses on delivering tenant-centered case management, coordinating access to community resources, and fostering a safe, supportive residential environment. The ideal candidate is skilled at balancing documentation, crisis response, and relationship-building while working with individuals facing behavioral health, substance use, and medical challenges.
