**Description:**
The Care Transitions Case Manager collaborates with physicians, social services, nurses, community agencies and other members of the care team to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Care Transitions Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition of patients through medically appropriate levels of care.
Schedule:
Monday - Friday
6:30 am - 3:00 pm
8 hour shifts
Rotating weekends and holidays
1 day off every other week
Qualifications:
+ **Current and valid license to practice as Registered Nurses in Iowa** , BSN or approved BA is required or willing to obtain within three years of employment
