Job Description
Become a vital part of the Neighbourhood Group (TNG) as a Case Manager for the Mobile Shelter Support Program. This full-time role involves providing supportive case management to adults in shelters and warming centres around the GTA.
The Case Manager will work directly with clients facing complex challenges, including housing, mental health, and legal issues. By utilizing a trauma-informed, harm-reduction approach, you will help participants set and achieve their self-directed goals through dedicated support. Collaborating with external service providers is crucial to deliver comprehensive resources and assistance to clients.

• Coordinate referrals and advocacy for various supports
• Facilitate ongoing community support and home visits
• Monitor housing issues and liaise with landlords
