Job Description
Our Client, a Healthcare company, is looking for a Case Management – RN for their Fairfield, CA location. Responsibilities:
+ The Inpatient Case Manager assess, plan, implement, monitor & evaluate options & services to affect an appropriate, individualized plan for hospital patient care across the continuum of care.
+ The Input CM will follow assigned patients working w/team members & Social Services to prioritize & strategize each day’s activities.
+ Using independent judgment & discretion, the patient must be assessed & reassessed as an ongoing process to address the patient’s broad spectrum of needs.
+ The Input CM utilizes clinical knowledge & competence, positive communication skills, problem solving & conflict resolution techniques, ability to effect change, strong skills in assessment, organization, & time management.
