Care Partner Support at Home - Elizabeth Location

Resthaven has a vacancy for a Care Partner Support at Home to join the Resthaven Community Services Team in providing  comprehensive coordination and individualized case management, and planning in partnership with Support at Home individuals.

Located at our Northern Community Site at Elizabeth (also working across our site at Gawler), this role is offered as a full-time permanent role. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes clear and respectful communication, active listening, and providing timely updates.
