We are currently recruiting for a full time Care Partner to be based in Bacchus Marsh with travel to main office in Ballarat and western Metro suburbs.
As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities. Your days will be filled with meaningful interactions, as you:
Deliver high-quality, person‑centred care to clients in their homes and communitiesConduct comprehensive assessments, develop care plans and coordinate servicesEmpower clients and carers to make informed choices about their careEnsure compliance with aged care quality standards and organisational policiesMaintain accurate case notes, documentation and client confidentialityWork collaboratively within a supportive, multidisciplinary team and escalate concerns where required About you...
