Responsibilities
- Ensure a supportive, positive experience for clients and referral sources
- Work directly with clients, families, and referral sources to understand needs
- Make accurate and timely outbound referrals for those not admitted
- Collaborate with internal clinical and admissions teams
- Document all client and referral source interactions in the electronic record system
- Function as a liaison between Charlie Health and partners
- Adhere to policies and achieve performance metrics goals
Requirements
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 1-2 years of relevant healthcare experience (e.g., case management, admissions, or outreach)
- Strong interpersonal and relationship-building skills
- Metrics-oriented mindset with experience meeting KPIs
