Overview

The Behavioral Health Care Navigator provides specialized expert support, intervention, and care coordination to members at high risk of adverse health outcomes specific to mental and behavioral health needs. When members are identified through risk stratification or referred from the community, the Behavioral Health Care Navigator follows established protocols, adhering to strict timelines and documentation standards to perform an assessment and engage the member and caregiver in maximizing systems and resources to meet the needs in the least restrictive environment. The Behavioral Health Care Navigator pays special attention to member safety and provides education to caregivers on safety planning.
