Role Summary

Would you like to be part of our dedicated team of a multi-disciplinary team who help people to stay safe and independent in their own homes or recover from injury and illness in our local community? We are looking for a Band 4 AP to cover maternity leave for a period of months to join our team of Occupational Therapists, Physiotherapists and Nurses who make up the Central & East Exeter Urgent Community Response Team.

The UCR team provide an urgent response to health and/or social care needs whether they are at risk of an admission to hospital or to facilitate discharge home from hospital. 

You will be visiting patients within their own home environment to assist with assessments and carry out treatment, with a goal-orientated focus, within your competencies or as delegated by registered staff.
