Assistant case manager

Case Management is described by The Case Management Society UK as a collaborative process which: assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individuals health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes.

You will be providing a Case Management service to our clients that is devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. This will be done within the ethos of the company
